Doing my part to keep DMG and my favorite gymnast and/or dancer safe.

 

I want to do my part to help DMG keep my child(ren), her teammates, the coaches, other families and everyone else at the gym as safe as possible under the Covid-19 pandemic. I have read, understood and agree to follow the following policies and procedures. 

 

I understand and agree that:

• Drop-off and Pick-up

  • DMG 1: I will drop my child off at the front door and pick them up at the back door to the gym for team kids, and recreational students will leave with parent at front door.

  • DMG 2: I will drop off and pick up my child at the front door. 

• Only one parent or non-participating individual (no siblings) will be allowed to enter the building at the main entrance. Gymnastics Team and Dance team parents please drop off and pick up.

• I am, as a parent, have the option of wearing a mask. This can change at any time.

• I am aware that my child may wear a mask but that he/she is not required to do so unless the teacher or coach asks them to due to close contact. If your child is required to wear a mask during class per the instructor’s request, but does not have a mask with them, they can receive one from the front desk for $1.00 (this will be charged to your account).

• Each child that enters DMG will need to bring their own water bottle that is labeled with their name. All water fountains will be turned off. If your child forgets a water bottle, they can get one from the front desk and it will be charged to your account ($1.00).

• Viewing area will be limited and I might have to drop my child off.  

• There will be hands-on spotting for gymnastics and tumbling classes for the safety of your child. If you do not want your child to be spotted, please notify us by email at dmgdesk82@gmail.com.

• I will support the social distancing standard of 3’ while at the gym. 

• TEAM gymnasts and dancers will provide their own hand sanitizer in their gym bags. Recreational gymnasts and dancers will have regular opportunities to use the hand sanitizing stations available in the facility.

• My child will use the restroom and wash her hands thoroughly before leaving home and while at the gym as needed. I will have my child use the restroom and be already dressed upon entering the gym.

• My TEAM gymnast or dancer will bring her clearly marked bag to the gym each day with all recommended items included.

• I agree to keep my gymnast home if he/she or anyone in my family is coughing, has a temperature over 100.4, or any other Covid-19 symptoms. If your dancer or gymnast is showing signs of illness, they may be asked to put a mask on. 

• I understand and agree that these procedures will change and evolve over time and that I will follow any new standards required by the State/County and/or DMG.

• If my child gets sent home from school and/or has been quarantined due to close contact with someone who tested positive for Covid-19, we will follow the same rules and guidelines as TSC school corporation. If this happens to your child, please contact us so we can further assist you.

• Makeups for missed classes: Every 8 weeks, there will one Saturday makeup opportunity for gymnastics and tumbling. Please keep an eye out for emails regarding this.

 

ASSUMPTION OF RISK/COVID-19 

I acknowledge that sports and activities involving height, motion or inversion including but not limited to gymnastics, trampoline, cheerleading, and dance carry the risk of severe injury, including paralysis or death. I acknowledge the contagious nature of certain bacteria and viruses, including, without limitation, COVID-19, and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by such bacteria or viruses by attending DMG and that such exposure or infection may result in personal injury, illness, permanent disability or death. I understand that the risk of becoming exposed to or infected by COVID-19 at DMG may result from the actions, omissions, or negligence of myself and others, including, but not limited to, DMG employees, volunteers, and program participants and their families.

I understand that the coaches and everyone at the gym will make a strong effort to maintain social distancing but that there will be times when incidental contact and less than prescribed physical distancing will occur. I am aware and agree that spotting is an essential part of training my gymnast in order to keep your child safe and to prevent injury. I will allow my child to be spotted when spotting is necessary. I further understand that I am voluntarily allowing my child to participate in programs and activities offered by DMG Inc., knowing that it is impossible to keep him/her, myself or anyone else who enters the gym completely safe from exposure to the Covid-19 virus. I accept that risk.

I VOLUNTARILY AGREE TO ASSUME ALL OF THE FOREGOING RISKS AND ACCEPT SOLE RESPONSIBILITY for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s or my attendance at DMG or participation in DMG programming.
